Foraminiferal analyses of 404 contiguous samples, supported by diatom, lithologic, geochronologic and seismic data, reveal both rapid and gradual Holocene paleoenvironmental changes in an 8.21-m vibracore taken from southern Pamlico Sound, North Carolina. Data record initial flooding of a latest Pleistocene river drainage and the formation of an estuary 9000 yr ago. Estuarine conditions were punctuated by two intervals of marine influence from approximately 4100 to 3700 and 1150 to 500 cal yr BP. Foraminiferal assemblages in the muddy sand facies that accumulated during these intervals contain many well-preserved benthic foraminiferal species, which occur today in open marine settings as deep as the mid shelf, and significant numbers of well-preserved planktonic foraminifera, some typical of Gulf Stream waters. We postulate that these marine-influenced units resulted from temporary destruction of the southern Outer Banks barrier islands by hurricanes. The second increase in marine influence is coeval with increased rate of sea-level rise and a peak in Atlantic tropical cyclone activity during the Medieval Climate Anomaly. This high-resolution analysis demonstrates the range of environmental variability and the rapidity of coastal change that can result from the interplay of changing climate, sea level and geomorphology in an estuarine setting.

Using computer simulations and analytic calculations, we have evaluated whether conspicuous expansions and contractions of natural clades may have represented chance fluctuations that occurred while probabilities of speciation and extinction remained equal and constant. Our results differ from those of previous workers, who have not scaled generating parameters empirically at the species level. We have found that the waxing and waning of many real clades have almost certainly resulted from changes in probabilities of speciation and extinction. For some of these changes, likely explanations are evident. The emplacement of adaptive innovations, for example, has at times elevated probability of speciation. We conclude that chance factors have not played a dominant role in producing dramatic changes in standing diversity within speciose higher taxa.

The cognitive profile of early onset Parkinson’s disease (EOPD) has not been clearly defined. Mutations in the parkin gene are the most common genetic risk factor for EOPD and may offer information about the neuropsychological pattern of performance in both symptomatic and asymptomatic mutation carriers. EOPD probands and their first-degree relatives who did not have Parkinson’s disease (PD) were genotyped for mutations in the parkin gene and administered a comprehensive neuropsychological battery. Performance was compared between EOPD probands with (N = 43) and without (N = 52) parkin mutations. The same neuropsychological battery was administered to 217 first-degree relatives to assess neuropsychological function in individuals who carry parkin mutations but do not have PD. No significant differences in neuropsychological test performance were found between parkin carrier and noncarrier probands. Performance also did not differ between EOPD noncarriers and carrier subgroups (i.e., heterozygotes, compound heterozygotes/homozygotes). Similarly, no differences were found among unaffected family members across genotypes. Mean neuropsychological test performance was within normal range in all probands and relatives. Carriers of parkin mutations, whether or not they have PD, do not perform differently on neuropsychological measures as compared to noncarriers. The cognitive functioning of parkin carriers over time warrants further study. (JINS, 2011, 17, 1–10)

Summary
The literature on emerging adulthood (EA) has focused heavily on basic research, a reflection no doubt of continuing debate about fundamental features of this stage of development (see the special issue of Child Development Perspectives, 2007). Therefore it is not surprising that examination of applied issues relating to romantic relationships in EA has been limited to documenting an association between romantic relationships and psychological adjustment (see Chapter 14) and to showing that the quality of such relationships mediates the link between earlier parent–adolescent relationships and later mental health outcomes (Beyers & Seiffge-Krenke, 2007). The purpose of this chapter is to broaden attention to applied issues in the EA literature by considering how to facilitate what seems to be a prerequisite to entering adulthood: the ability to initiate and sustain an intimate romantic relationship with another person.
WHAT IS RELATIONSHIP EDUCATION AND IS IT NECESSARY?
Historically, relationship education has taken the form of marriage education and has been offered almost exclusively to couples. Such marriage education has its roots in religious institutions, and the precursor to modern programs was Pre-Cana premarital counseling now required of couples wishing to marry within the Catholic Church (Ooms, 2005). Until quite recently, most relationship education occurred in religious contexts, thereby excluding those who do not profess a religious faith or who are uncomfortable in such settings. Nonetheless, a substantial minority of marrying couples (44%) in the 1990s received some type of relationship education before marriage (Stanley, Amato, Johnson, & Markman, 2006).

Summary
Cohabitation has become a common element of the path that emerging adults travel as they navigate romantic relationships before settling down into marriage. There has been an explosion in the practice of cohabiting before marriage, and many emerging adults now cohabit regardless of marriage intentions. Along with the wide availability of birth control and the increased likelihood of nonmarital sex, cohabiting reflects fundamental changes in how men and women date and mate. This chapter describes this growing trend, including how individuals during the period of emerging adulthood view cohabitation and how they can be affected by it as they pursue their romantic relationship and family goals.
LIVING TOGETHER OUTSIDE OF MARRIAGE: A NEW NATIONAL TREND
Many changes have occurred in family demography in the United States over the past several decades. One of the most significant changes is an increase in the number of couples who live together without being married. The U.S. Census now inquires about romantic partners sharing a household, and the last report indicated that unmarried couples were living together in 4% to 6% of U.S. households. This upward trend in cohabitation is likely linked with many other changes in recent decades, including the diminishing number of U.S. households that involve married couples and, most significantly for the focus of this book, the trend toward delaying marriage until the late 20s.

Background
There are claims that second-generation antipsychotics produce fewer extrapyramidal side-effects (EPS) compared with first-generation drugs.
AimsTo compare the incidence of treatment-emergent EPS between second-generation antipsychotics and perphenazine in people with schizophrenia.
MethodIncidence analyses integrated data from standardised rating scales and documented use of concomitant medication or treatment discontinuation for EPS events. Mixed model analyses of change in rating scales from baseline were also conducted.
ResultsThere were no significant differences in incidence or change in rating scales for parkinsonism, dystonia, akathisia or tardive dyskinesia when comparing second-generation antipsychotics with perphenazine or comparing between second-generation antipsychotics. Secondary analyses revealed greater rates of concomitant antiparkinsonism medication among individuals on risperidone and lower rates among individuals on quetiapine, and lower rates of discontinuation because of parkinsonism among people on quetiapine and ziprasidone. There was a trend for a greater likelihood of concomitant medication for akathisia among individuals on risperidone and perphenazine.
ConclusionsThe incidence of treatment-emergent EPS and change in EPS ratings indicated that there are no significant differences between second-generation antipsychotics and perphenazine or between second-generation antipsychotics in people with schizophrenia.

Improved Perioperative Antibiotic Use and Reduced Surgical Wound Infections Through Use of Computer Decision Analysis

A prospective study was performed over a two-year period to determine whether computer-generated reminders of perioperative antibiotic use could improve prescribing habits and reduce postoperative wound infections. During the first year, baseline patterns of antibiotic use and postoperative infection rates were established. During the second year, computer-generated reminders regarding perioperative antibiotic use were placed in the patient's medical record prior to surgery and patterns of antibiotic use and postoperative wound infections monitored.
Hospitalized patients undergoing non-emergency surgery from June to November 1985 (3,263 patients), and from June to November 1986 (3,568) were monitored with respect to indications for perioperative antibiotic use, timing of antibiotic use and postoperative infectious complications. Perioperative antibiotic use was considered advisable for 1,621 (50%) patients in the 1985 sample and for 1,830 (51%) patients in the 1986 sample. Among these patients, antibiotics were given within two hours before the surgical incision in 638 (40%) of the 1985 sample and 1,070 (58%) of the 1986 sample (p<0.001). Overall, postoperative wound infections were detected in 28 (1.8%) of 1,621 patients in 1985 compared with 16 (0.9%) of 1,830 such patients in 1986 (p<0.03).
We conclude that computer-generated reminders of perioperative antibiotic use improved prescribing habits with a concurrent decline in postoperative wound infections.
